Services

No appointment is ever needed when we are in the office. Check the vacation and holiday schedule before you come — call (508) 540-6790.

We accept checks.

Immigration Medical ExamsI-693 · one-stop visit

Dr. Lee is a certified Designated Civil Surgeon, licensed by U.S. Citizenship and Immigration Services. He performs your immigration exam, does all the necessary testing, updates your vaccine requirements and completes your I-693 form — all in the same visit, so you avoid going from place to place on different days.

Bring with you

  • Passport or driver’s license
  • Any medical records you have, especially past vaccine records (no need to worry if you have none)

Photos are not needed.

What happens

Dr. Lee reviews your history and records, draws blood and collects a urine sample for the required testing, tests for vaccine protection and administers any vaccines USCIS requires. Hepatitis B vaccine is required as of May 2022, polio vaccine as of May 2024, and a TB blood test is now required instead of skin testing. When results and vaccines are complete, Dr. Lee certifies your I-693; return in 2–5 days to pick it up and send it to USCIS. For Martha’s Vineyard, Nantucket and Lower Cape residents, we may be able to mail you the form and save a second trip.

DOT & Coast Guard ExamsCDL medical cards · CG-719K

DOT physical. Dr. Lee is a Certified DOT Medical Examiner. Come in any time during regular hours — Mon–Tue 9 AM–4 PM, Wed–Thu 9 AM–12 PM. Bring any forms and related medical reports; a urine sample is checked as part of the exam. After a successful exam, Dr. Lee validates your CDL medical card with the National Registry and the Massachusetts RMV right away.

Coast Guard exam. We specialize in the CG-719K form, completing the entire form in one sitting. Your Coast Guard drug test can be done at the same time, for a hassle-free visit.

Drug TestingMRO · NIDA 9/12 panel

Dr. Lee is a certified Medical Review Officer, licensed to conduct and certify drug test results for the Coast Guard license exam and for CDL drivers under the Department of Transportation. He runs the NIDA 9/12 panel through SAMHSA-approved labs; other drug panels are also available. The office is a collection site for many companies with CDL drivers and marine employees.

Travel Clinic & VaccinesThe Cape’s only board-certified travel medicine specialist

Dr. Lee is the only Board Certified travel medicine specialist on the entire Cape, certified by the International Society of Travel Medicine. Available vaccines include Yellow Fever, Hep A, Hep B, Typhoid, injected Polio, Tetanus-Diphtheria, Tdap, PPD, MMR, Meningitis and Varicella. Japanese Encephalitis and Rabies vaccines are in stock, and the Chikungunya and cholera vaccines are now available.

As part of your pre-travel consultation you receive the appropriate prescriptions for malaria prevention, traveler’s diarrhea and altitude sickness, plus advice to stay healthy and safe on your trip. No appointment is needed for the consultation or the vaccines.

Pre-Op, Work & Sports PhysicalsBring your form

Pre-op, work and sports physicals of all kinds. Just come during regular hours and bring the form your specialist, employer or school gave you. We can also do labs and give any required vaccines in the same visit.

Suboxone TreatmentBoard-certified addiction medicine

Dr. Lee is a Board Certified Addiction Medicine Specialist and a licensed Suboxone prescriber, offering comprehensive outpatient treatment for opioid use disorder. The program is built around individual needs, with a heavy emphasis on exercise and counseling. No appointment is needed for the initial visit; there may be a waiting list before evaluation and treatment begin.

Once accepted, you are seen frequently for follow-ups. To check compliance, we review the Mass Prescription Monitoring Program and do random drug testing. Successful addiction treatment starts from the understanding that this is a chronic medical brain disease; for almost all patients, long-term sobriety comes from a healthy lifestyle and long-term use of medication.

What patients say

Reviews

I’ve been seeing Dr. Lee for years, given his multiple licenses, degrees and certifications I feel very lucky to be able to depend on someone so qualified (if not overqualified). Whenever I have a question, he goes above and beyond to explain medical concepts to me in great medical detail in addition to ways that make sense to me as a simple person. Would definitely recommend.
